Floor Claim Registration

1. Floor Claim Registration must be completed & submitted to our claim department:

david.nguyen@uniquewoodproducts.com, if there is insufficient information, the form will be returned for completion. Images and/or pictures are required for all claims filed. VERBAL REPORTING OF AN ISSUE DOES NOT CONSTITUTE FILING A CLAIM. NO ACTION WILL BE TAKEN UNTIL A COMPLETE CLAIM FORM IS RECEIVED BY OFFICE.

2. Unique Wood Products reserves the right to request written job site pre-installation documentation.

3. Upon receipt of all required documents, a claim number will be issed to you. All claims shall begin the processing pase within 10 business days of receipt of completed form. This claim number should be used for all corespondence toward this claim, we will begin to review and process your claim.

4. You will be notified by writing or email under any of the following conditions:
a. The claim is approved and how Unique Wood Products propose to resolve this issue
b. The claim has been denied and our basis for such denial.
c. We are exercising our right to have the flooring examined by an independent inspector

It generally takes 14-21 days to received a written report for a NWFA inspector. Should we determine an independent inspector is warranted, the person filling out this claim shall be responsible for any inspection fees regarding this claim, if determined the material / nonmanufacturing related is defective, Unique Wood Products will reimburse the fee to the person filling out this claim. Settlements of claims for manufacturing defects will be in accordance with the NWFA inspector and conclusion/cause based on the facts, testing, observation and industry guidelines as supported through the NWFA Inspection Report Form
