UNIQUE WOOD PRODUCTS the authority to process payment for all orders placed via fax, phone, or email, using the credit card specified
above. I acknowledge that I am the rightful cardholder and assume full responsibility for all transactions associated with my account.
Furthermore, I understand and accept that UNIQUE WOOD PRODUCTS reserves the right to terminate any outstanding orders at their
discretion if charges are declined or if any chargebacks are initiated against invoiced amounts. Any discrepancies regarding invoiced
amounts must be promptly reported to INFO@UNIQUEWOODPRODUCTS.COM. I am committed to promptly notifying UNIQUE WOOD
PRODUCTS of any changes to the status of the specified credit card.
I hereby authorize UNIQUE WOOD PRODUCTS to charge the aforementioned credit card in the event that my account becomes delinquent
for more than 30 days.
The undersigned is the duly authorized person on the above credit card.
By signing your name above, you agree your electronic signature is the legal equivalent of your manual signature on this form. Be advised there are no returns or chargebacks credits given on any custom or special order material. Also clicking on the Submit button below, you agree that if, after payment by credit card, you later dispute the charges, unless prohibited by law, you agree not ot cancel, revoke chargeback or dispute any previously entered charge on your credit card. If you do so, and it is later determined that the charge was properly authorized, you agree to pay all out-of-pocket fees and cost incured by Unique Wood Products as a result of improper, cancellation, revocation and charge back or dispute.
